School Immunization Requirements

Measles

First dose required on or after 
the first birthday 
or 
If enrolling in Kindergarten on or after August 2000, 2 doses are required at least 30 days apart, 
the first on or after the first birthday
 or 
Is or has enrolled in 7th grade after September 1992, 2 doses are required, the first on or after the first birthday.

mumps

One dose required 
on or after the first birthday.

rubella

One dose required 
on or after the first birthday.

polio

3 doses of polio vaccine so long as  4 weeks between each dose
 or 
For those enrolled in K-12, at least 1 dose of polio vaccine must be given 
on or after fourth birthday.

diphtheria /tetanus/  pertussis (dtp / dtap / dt / td)

4 doses of diphtheria, tetanus, and pertussis toxoid so long as 3 doses are given at least 4 weeks apart and the fourth dose is given at least 
6 months after the third
or 
For those enrolled in K-12, at least 1 dose must be given 
on or after the fourth birthday
or 
For those 72 months of age or older, 2 doses of tetanus, diphtheria toxoid (td) at least 4 weeks apart followed by a third dose at least 6 months after the 2nd and on or after the fourth birthday.

haemophilus influenza
type b (h
ib)

1 dose of Hib vaccine given 
at age 12 months or older 
and before age 5 years.

hepatitis B

For those born January 1, 1994 or later, 3 doses of Hepatitis B vaccine are required with 2nd dose given at least 4 weeks after 1st. Third dose must be given at least 4 months after 1st dose and at least 2 months after second dose.
 or 
For those born before January 1, 1994 and enrolled in 7th grade in August 2000 or later, 1 dose prior to start of school
or 
For those born before January 1, 1994 and enrolled in 8th grade in August 2001 or later, 3 doses prior to start of school with at least 21 days between dose 1 and 2, at least 2 months between dose 2 and 3, and at least 4 months between 1 and 3.

varicella (Chicken Pox Vaccine)

1 dose required on or after first birthday for those born after 1/1/97. Also required for those enrolled in 7th grade as of August 2000 or later. 
2 doses at least 4 weeks apart are required for those receiving the first dose after their 13th birthday
 or 
Students will also be considered adequately protected against varicella if they have a written statement signed by a physician, physician assistance, or nurse practitioner indicating a history of varicella.
