COVID-19 Vaccine Phase Update 

                            TAHD is currently providing the Moderna COVID-19 vaccine for the groups in PHASE 1A.

Phase 1a individuals include:

Those eligible for vaccine are:

Healthcare Personnel: All paid and unpaid persons serving in healthcare settings who have the potential for direct or indirect exposure to patients of infectious materials.

Long Term Care Facility Residents: Adults who reside in facilities that provide a range of services, including medical and personal care, to persons who are unable to live independently.

First Responders at risk of exposure to COVID-19 through their response to medical emergencies such as Emergency Medical Technicians, Police, and Fire

Phase 1a does not include healthcare personnel that do not have the potential for direct or indirect exposure to patients or infections materials. For example, if you are providing telehealth services and do not currently work in a healthcare setting providing in person care, you are not be eligible for vaccine in phase 1a.

​Phase 1b individuals eligible at this time include:

  • Individuals 75 years of age and older 

  • Front line essential workers 

  • Individuals and staff in congregate settings

PHASE 1B ELIGIBILITY INFORMATION
Phase 1B vaccinations will start once all Phase 1A groups are completed and further directions are given from the state. 
​It is currently not yet known when Phase 1B or further phases will begin. 

​COVID19 Vaccination Survey

THIS FORM IS FOR TORRINGTON AREA HEALTH DISTRICT RESIDENTS ONLY!
The survey is to enable registration for COVID-19 Vaccination for those eligible at this time.  This survey enables the Health Department upload your initial information into the Vaccine Administration Management System (VAMS) which is the system you will use to register for vaccination. Within 24 hours, each individual will receive an email from CDC.GOV/VAMS providing instructions to register in VAMS, complete the required medical forms and to select a vaccination appointment.

TAHD can not administer the COVID-19 Vaccine if...

  • You've received any vaccine in the past fourteen (14) days.
  • You've had a severe allergic reaction (e.g., anaphylaxis) to something. For example, a reaction for which you were treated with epinephrine or EpiPen, or for which you had to go to the hospital. 

If you've experienced any of the items listed above please CANCEL any appointments made with TAHD.
